Hospital Cash

A daily benefit of $20.00 will be payable to you, the member, when such insured member is in a hospital and under the
regular care and attendance of a physician, but only in such period of hospitalization:

1. Is necessary for the treatment of injury and,
2. Begins while insurance under the policy is in force as to that member.

The daily benefit will be paid from the first day of hospitalization (one night admission), but in no event for more than
three hundred and sixty-five (365) days per injury or sickness, provided the member is in hospital and under the care
of a physician.

Recurrent Disability

If a particular condition causes more than one period of hospitalization due to the same or related causes, then the
maximum benefit of three hundred and sixty-five (365) days in a hospital will be reinstated, provided a period of one
hundred and eighty-three (183) days have elapsed between periods of hospitalization.

Exclusions

The policy does not cover any period of hospitalization caused or contributed by:

1. Declared or undeclared way or any act thereof;
2. Active full-time service in the armed forces of any country; or
3. Cosmetic treatment or surgery (except when neccessary due to injury caused by an accident).

Submitting a Claim

1. Complete the Hospital Cash Plan Group Claim Form by Medavie Blue Cross.  All sections of the form must be completed to ensure timely processing.  The claim form is available here - https:// (English) or https:// (French.
2. Once completed, submit the claim form to belairdirect, our Plan Administrator, for processing.
3. Claims may be submitted by mail or email using the contact information below:

Claims must be submitted within one year from the date the expense was incurred.

This benefit is paid for by the NSTU Group Insurance Trust Fund.
